After signing an agreement with Renaissance Health Systems of Florida, Inc., the next step to become an official Renaissance Provider is to complete our credentialing process. Primary Care Physicians (PCP's) and Specialists must fill out the Credentialing Application, Provider Profile, and the W-9 form. For Ancillary Services, the Ancillary Services Application and the W-9 must be filled out. Our qualified team will review your application and upon approval will contact your office to schedule an on-site visit.
